(B) the division(s) in which you want your team(s) to play
(C) a name & phone number where you can be reached for questions
2) Submit your team's Official Roster

3) Submit your team's Entry Fee
Submit your check or money order payment by mail. We must receive your payment no later than May 17th, 2010.
